Car accidents are the most common cause of traumatic disc herniation. Understand your diagnosis and how physical therapy can help you recover without surgery.
A herniated disc is one of the most serious and most commonly misunderstood car accident injuries. The intervertebral disc acts as a shock absorber between each vertebra in your spine. During the violent forces of a collision, the outer fibrous ring (annulus fibrosus) can tear, allowing the gel-like center (nucleus pulposus) to bulge outward and compress nearby nerves. This can happen in the neck (cervical disc herniation) or lower back (lumbar disc herniation), and the resulting nerve compression produces some of the most disabling symptoms of any car accident injury.
Dynamic PT specializes in disc herniation rehabilitation at our Glendale, Van Nuys, and Woodland Hills clinics. We accept personal injury medical liens - you begin treatment immediately with no money out of pocket.
Cervical Disc Herniation from Car Accidents
The cervical spine is most vulnerable to disc herniation at C4-C5, C5-C6, and C6-C7. A rear-end collision that hyperflexes and hyperextends the neck can rupture one or more of these discs. The compressed nerve root produces symptoms in the arm and hand that correspond to the dermatome and myotome of the affected level.
- C4-C5 herniation: shoulder pain and deltoid weakness
- C5-C6 herniation (most common): pain and numbness into the thumb and index finger, bicep weakness
- C6-C7 herniation: pain and numbness into the middle finger, tricep weakness
Lumbar Disc Herniation from Car Accidents
The lumbar spine absorbs the compressive and shear forces of impact, particularly at L4-L5 and L5-S1. A traumatic lumbar disc herniation at these levels irritates the L4, L5, or S1 nerve roots, producing classic sciatica - radiating pain, numbness, and weakness running from the lower back through the buttock and down the leg.
Why PT Often Avoids Surgery
The majority of disc herniations, even significant ones, can be successfully managed conservatively with physical therapy. Disc material can gradually resorb over several months, and targeted PT accelerates this process while reducing nerve sensitivity and restoring function. Surgery is typically reserved for cases with progressive neurological deficit, cauda equina syndrome, or failure of conservative treatment after 3 to 6 months.
Physical Therapy Protocol for Disc Herniation
Phase 1: Pain Control and Nerve Calming (Weeks 1-4)
The initial focus is reducing nerve irritation and inflammation. We use manual therapy, traction (cervical or lumbar), modalities such as electrical stimulation and ultrasound, and directional preference exercises (McKenzie method) to centralize and reduce pain.
Phase 2: Mobility and Stabilization (Weeks 4-10)
As pain decreases, we introduce range of motion work, core stabilization, and neuromuscular control exercises. For cervical disc injuries, deep cervical flexor training is key. For lumbar injuries, the transverse abdominis and multifidus are the primary targets.
Phase 3: Functional Rehabilitation (Weeks 10+)
The final phase prepares you to return to work, driving, and daily activities. Functional movement patterns, endurance training, and posture correction complete the rehabilitation program and reduce the risk of re-injury.
MRI Documentation
We strongly recommend obtaining an MRI if a disc herniation is suspected. MRI findings combined with our clinical documentation create a strong medical-legal record for your personal injury claim. We can help coordinate referrals to imaging centers that work on lien.
Lien-Based PT for Disc Herniation in California
Disc herniations from car accidents are among the most well-compensated injuries in personal injury cases because they are objectively documented and result in significant impairment. Dynamic PT provides the clinical treatment and comprehensive documentation that personal injury attorneys rely on to accurately represent the extent of your injuries.
